What You Will Do
• Coordinate with families, nurses, therapists and providers of residents needs according to the care plan
• Assist in discharge planning or transitions to other levels of care
What We Are Looking For
• Bachelors degree in Social Work
• 1-3 years of social services experience in a healthcare setting
