We are looking for a motivated and caring Client Care Coordinator to join our fast-growing team and play a key role in ensuring the highest standards of care for our clients. 

Role Overview 

The Client Care Coordinator is the main point of contact for clients and their families, responsible for developing care plans, ensuring quality service delivery, and supporting our Care Professionals. This role is ideal for someone with excellent communication skills, strong organizational abilities, and a genuine passion for improving the lives of others. 

Key Responsibilities 

  • Conduct care consultations, assessments, and regular quality assurance visits. 
  • Build strong relationships with clients and their families, ensuring satisfaction and wellbeing. 
  • Develop, implement, and review personalized care plans. 
  • Support Care Professionals with guidance, training, and ongoing communication. 
  • Work with the scheduling team to ensure the right Care Professionals are matched with clients. 
  • Maintain accurate documentation and compliance. 
  • Represent Home Instead in the local community and strengthen referral relationships. 

What We’re Looking For 

  • Experience in health and social care, ideally in a care coordination or supervisory role. 
  • Knowledge of person-centered care and safeguarding principles. 
  • Strong communication and organizational skills. 
  • Compassion, empathy, and professionalism in all interactions. 
  • Professionals with an ownership mindset who demand results and have a take-action attitude.
